The Angels Community Clinic of Murray and Calloway County was established in 2000 to provide diagnostic and treatment services to those people who do not have medical insurance or financial resources necessary to pay for private insurance and/or medical care. Services of the Health Department will not be duplicated.
The service by the Angels Community Clinic will be provided to legal residents of Murray and Calloway County.
Free care is offered with mutual dignity and respect to those at or below 200% of the federal poverty level. Income information to be included will be limited to that earned by members of the household who are related and/or are dependents.
Persons living in the home who are not related and are not dependents will not be considered to qualify nor disqualify others in the homes. Examples:
a) A person who works and lives with a significant other who also works is qualified even if the second income would exceed the maximum for family size;
b) A person who has no earned income is not qualified although he/she lives with a significant other who has earned income and may qualify for services.
An adult legal resident of Calloway County who attends a school full time but does not work is qualified even if there are no employed persons in the household. Services of the Murray State University Student Health Services will not be duplicated.
Not eligible for service:
a) Persons with medical insurance
b) Persons with military identification
c) Persons with Medicare, Medicaid and those on Workers Compensation
d) Unemployed persons who have not worked within the last six months, or whose spouse has not worked in the last six months
e) Persons whose income exceeds the maximum amount for family size as listed above.
Services of the Angels Community Clinic are available to all those in the identified service area who meet the established financial guidelines. No person shall be denied service on the basis of race, religion, national origin, handicap, or sex. Wheelchair access to the clinic is maintained.
Who is the clinic for?
The Clinic is designed to help adult men and women who have jobs, or have been laid off within the past six months and currently reside in Calloway County. These are people who are not covered under health insurance of any type.
What must I bring to the Clinic when I come?
You must bring the most recent pay stubs or notarized verification of employment of all working family member(s) each visit. You must also have documentation of any other household income (SSI, Disability, Food Stamps, Child Support, etc.)
If you are self employed you will also need to bring schedule 1040 C or E forms.
Forms of ID that are accepted are a Calloway County driver's license or a State Issued Photo ID.
Patients should bring all the prescription and over the counter medications that they are currently taking so the Doctor will know exactly what medications they are taking.
What is the screening process?
The patients will be screened from 4:00 PM - 6:00 PM and if you meet the eligibility requirements, you will be seen that night. Angels Community Clinic income guidelines are as follows:

Remember, it is required that each time you come to be seen by a Doctor, Dentist or to pick up medications that you bring your most current pay stubs or verification of employment form signed by your employer for all working household members.
